The survey is split into sections. Please answer the questions that match your own journey. You can skip any questions that don’t apply to you.
Some questions talk about sensitive things, like loss and bereavement. You can choose how much you want to share.

The MNVP is here to listen and use feedback to make meaningful changes to our local maternity and neonatal services. However, if you wish to make a formal complaint about the care you received, you can contact the Patient Advice and Liaison Service (PALS) Team.
